Important Privacy Choices for Consumers

At the FABA Group (“we”) value the trust you have placed in us, and we are committed to earning and maintaining your confidence. We understand there is certain information you do not want to share with everyone, and, we share your concern about safeguarding the information you provide to us. This is why we have adopted this privacy policy.

This Privacy Policy is provided to you by the FABA Group. For convenience, the words “you,” “your,” or “yours” refer to each individual who is one of our customers or former customers and other individuals who obtain or request a health care solution or service from us.

We collect information about you that we believe will be necessary or useful in processing or administering products or services that you have requested from us. We will also collect information in order to notify you about products, services and other opportunities we think will be of interest to you. This information may be collected from a variety of sources, such as the following: information provided by you in applications or other online or PDF forms, and information about your transactions with us, our affiliates or others.

Our Privacy Principles

  • We do not allow those who are doing business on our behalf to use our member information solely for their own marketing purposes.
  • We do allow some entities that are willing to promote discounted services to members the ability to promote these services to our members, but only upon agreement that our members information will be protected.
  • We contractually require any person or organization providing products or services on our behalf to protect member’s information.
  • We do not sell or lease any member information to any outside source.
  • The FABA Group takes security of your information very seriously and has several methods for protecting information we collect. Some of these include: physical and logical access controls, firewalls and intrusion protection systems, file monitoring and backup, etc.

Information we collect

We collect information in order to better serve our members and for new customers to inquire about our services. Sometimes the information we collect includes simple things like what browser you are using. Sometimes the information we collect can also include sensitive or private information, such as health information and prescriptions. The FABA Group is always transparent about the information collected and why we collect it, and how we use the information.

  • Information you give us. This includes uploading images, files, and entering in personal information on an application or simply requesting more info. This type of information is always given to us voluntarily, and many times the purpose is to provide a service to you.
    • Information such as your name, address, date of birth, email etc. may be collected and used to send you additional information about the FABA Group or as part of your membership information. It is the goal of the FABA Group to only collect data necessary to conduct the business and any agreed upon service in a way that is simple and secure.
  • Information we get from your use of our services. We collect information about what you use on our site and how you use it. This information helps us to provide a higher level of service and gives us insight on how best to improve. The information we collect includes:
    • Log information When you use our services or view content provided by the FABA Group, we may automatically collect and store certain information in server logs. This may include:
      • Search queries or other fields you fill in
      • Internet Protocol Addresses (or IP addresses)
      • Device event information such as crashes, system activity, hardware settings, browser language, the date and time of your request and referral URL.
    • Local storage We may collect and store information (including personal information) locally on your device using mechanisms such as browser web storage (including HTML 5) and application data caches. This may be done to increase speed or security.
    • Cookies and anonymous identifiers We use various technologies to collect and store information when you visit  the FABA Group website and this sometimes includes the use of cookies. We also use cookies and anonymous identifiers when interacting with services from some third parties.
    • Other Information Some information is automatically recorded through tools such as Google Analytics.  does not always have direct control over what info is collected by Google Analytics, however where possible all information collected by Google Analytics or otherwise is protected in accordance with this policy.
    • Location information There are many methods to track location information. One method we utilize is IP address tracking which gives us the state and city you are connecting from. This helps us determine utilization and track issues and insure we are providing a high level of service to our members and guests.
    • Device information We may collect information about what hardware you are using. This may include whether or not you are using a mobile phone or a computer browser. Depending on the device used, information may include device number, browser version, operating system, etc.

Ensuring your information is as accurate, current and complete as possible, if you ever find that our records are inaccurate or not current, please let us know. If the record is within our control, we will correct any inaccurate or incomplete information.

We honor our customers’ privacy choices. For accounts with multiple customers we will apply an opt-out by any account holder to all account holders on the account. You have the right to control whether we share some of your personal information. You also have the right to control how that information is used. Please read the following information carefully.

Your Rights

You have the following rights to restrict the sharing of personal and financial information with our affiliates and outside companies that we do business with. You also have the right to limit our affiliates from marketing their products and services to you based on information that we share with them. Nothing prohibits the sharing of information necessary for us to follow the law, as permitted by law, or to give you the best service on your accounts with us. This includes sending you information about some other products or services. You may limit how we share your personal information. This is called a right to “opt out.” You have two options to consider.

  • Continue receiving valuable and convenient product and service offers
    You can enjoy the benefits of valuable product and service offerings without taking any action.
  • Opt out of information sharing outside of the FABA Group
    This choice would stop us from sharing non-public personal information with our carefully screened business partners. This may limit our ability to inform you of valuable and convenient products and services from partner companies.
